Receipt can’t be issued under the parents’ or caretaker;s name unless for tax relief purposes. Payment receipt can only be give to the receipients of treatment.

Medical Certificate (MC) are not allowed to be given over the counter. Patient are required to meet with the doctor for the MC
Maximum for 3 days (up to 5 days for special occasion, for example if major procedures in the treatment room). This shortens the time for follow-up and reduces medicolegal issues. It can’t be issued under the parents’ or caretaker’s name.
Timeslip can be given to both parents or caretaker. Time: 3 hours starting from the patient registration
1.What is an e-invoice?
E-Invoice is a digital invoicing system implemented by the Inland Revenue Board of Malaysia (LHDN) to streamline the invoicing process for business. It replaces traditional paper invoices with electronic invoices that are generated in specific formats like XML or JSON. This system aims to enhance efficiency, accuracy, and compliances with tax regulations by allowing real-time validation and transmission of invoices directly to LHDN for verification before reaching customers.
2. Why is e-invoice being implemented?
The implementation of e-invoicing by LHDN is aimed at promoting transparency, reducing tax evasion, and improving the efficiency of tax collection. By digitizing the invoicing process, LHDN can better monitor transactions, ensuring that business comply with tax regulations and that all sales are accurately reported.
3. When can I request an e-Invoice?
Malaysia has entered the e-invoice era starting 1 August 2024. Klinik As-Salam branches listed below will adopt e-invoicing under Phase 3, effective 1 July 2025. Customers may request an e-invoice within 72 hours of the visit date

4. How can I request an e-Invoice?
For walk-in customers:
Scan the QR code printed on your official receipt and complete the required details in the linked portal.
5. How will I receive the validated e-Invoice?
Your e-Invoice will be made available immediately upon successful validation by LHDN and can be downloaded by PDF format.

6. Can I cancel my requested e-Invoice?
Once an e-invoice request is usbmitted, it will be processed accordingly.
If needed you may reject the validated e-invoice within seventy-two(72) hours from the validation time, provided a valid reason i given. For amendments or further assistance, please contact our support team at 03-9078 3030
7. Who is eligible to request an e-invoice?
Only self-paying patients are eligible to e-invoices.
For panel patients, e-invoices will only be issued for out-of-pocket payments (eg. co-payment or items not covered by the panel)
8. Can a third-party payor (eg.company ) request an e-Invoice on behalf of the patient?
Yes. A payor company that bears the cost of treatment can request an e-invoice through the careline or directly at the clinic.

- Damaged or near-expired products within 30 days resulting from staff negligence.
- Staff malpractice and negligence in dispense medicine, providing services or procedures to customers.
- Services and/or medicine covered by a panel, but the patient is instructed to pay.
- Conditions allowable in accordance with Malaysian law and regulations.
Conditions Not Applicable for Refund:
- Cancellation of bookings; Please note that bookings are not transferable to other individuals or parties.
- Pre-payment for products or services will not be refunded if customers cancel their bookings after full or partial payment has been made and/or the procedure has not been completed, except for special cases with clinical management approval.
- Expiration of promotional services, such as vouchers, offers, and discounts, due to being unclaimed by the customers within the designated period.
- Other conditions not specified as eligible for refund.

1. Use of Panel
The use of the panel is non-transferable. Panels registered under an employee’s name cannot be utilized by dependents or any other individuals if they are not registered.
2. Patient Verification
Patients need to bring identification card, employee ID or company treatment form for verification purposes. If the patient fails to provide any valid identification document, the clinic reserves the right to deny the patient from using the panel services. This is to avoid claims not being paid by panel providers.
3. Over the Counter Medicine
Panel patients are not allowed to take over the counter medicine. Patients must see a doctor to avoid medication errors and comply with panel policy. Panel patients are also not allowed to take medication for use by others.
4. Long Term medication
Panel patients are not allowed to take long-term medication unless they are entitled to it. A maximum of 2 months of long-term medication is permitted.
5. Non covered medication/injections
Vitamins, supplements, probiotics, lotions, and cleansers are typically not covered by the panel unless the patient is entitled to this medication.
Requests for these items from patients are not permitted. However, prescriptions for these supplements under a doctor’s recommendation are allowed, and patients must bear the cost themselves.
6. Non covered procedure
The clinic will double-confirm with the panel provider for every procedure before it is carried out on the patient. Procedures not covered by the panel must be paid by the patient.
7. Medical checkup/Health Screening
The patient must obtain a Guarantee Letter (GL) from the company for any scheduled or routine health screening. The patient is responsible for the cost of medical check-ups not covered by the panel.
- If the projected price for medication or services communicated to the patient is significantly lower than it should be, the patient has the right to pay the initially quoted price.
- This policy ensure fairness and transparency in the communication of medication and service costs to patients.
The occurrence of infectious diseases needs to be reported to prevent their spread and control them from escalating into epidemics that could threaten human lives. Notification or mandatory reporting of infectious disease incidents to the authorities is provided for under the Infectious Diseases Prevention and Control Act 1988. The Control of Infectious Diseases Regulations, which were gazetted in 1993, also require the notification of 26 types of infectious diseases under that public health law, using a specific Notification Form.
What are the penalties for not reporting infectious diseases?
Under Section 25 of the Infectious Diseases Prevention and Control Act 1998, a compound of RM 1000 can be imposed by the Director General or Health Officer for the offense of not reporting an infectious disease incident. Section 24 under this act also imposes a General Penalty, and if convicted, the offender can be sentenced to imprisonment for not more than 2 years or a fine or both.

- The prospective pilgrim must not have undergone a Hajj health screening at another clinic. If you have already started a Hajj screening elsewhere and it is incomplete, please inform the original clinic to cancel the screening in the Hajj Health Examination Module via MyVAS (the online health screening system).
- Please bring along all your medical information and related records to help our healthcare staff process your screening smoothly.
- For pilgrims who have already completed their Hajj screening at another clinic and require a Specialist Doctor referral for a health condition, kindly make an appointment with the Specialist Doctor before coming to Klinik As-Salam.

How do I register a foreign worker and employer?
To register a foreign worker and employer, you need to visit (fomema2u.com.my) and complete the online registration process. Please note that foreign worker registration is not available at the FOMEMA headquarters. Visit the website and log in to the registration portal, where you can find options for appeals, service provider management, and support services.
If my worker is not fit during the assessment, can I appeal?
Yes, you can submit an online appeal to FOMEMA. FOMEMA will then determine whether an appeal can be initiated. You may also consult with the doctor who conducted the assessment by scheduling an appointment with them. The contact number for the doctor is 012 6382719 / 013 9969528. For FOMEMA appeal purposes, you can make a booking through KAS Booking online. The doctor in charge of appeals is Dr. Mohamed Neezar Mahadzir Bin Aris. The appeal process will be explained to you.
What preparations do I need for the FOMEMA Medical Checkup?
To prepare for the FOMEMA Medical Checkup, please follow these guidelines:
- Schedule an appointment between 9 am and 6 pm, from Tuesday to Saturday.
- Fast for 8 hours before the examination and ensure that your bladder is full. You may drink plain water and inform the clinic about any medications you are currently taking.
- Bring the FOMEMA Examination Form to the designated Panel Clinic, Laboratory, and X-Ray Center.
- Bring a valid travel passport for identification. Please note that only a copy of the passport is not allowed.
Can I go to a different branch of Klinik As-Salam other than the one in Bangi?
No, you cannot visit any other branch of Klinik As-Salam for FOMEMA purposes. Employers need to go to the Klinik As-Salam registered with FOMEMA as a Panel Clinic, which currently refers to the one in Bangi.
What types of jobs are suitable for FOMEMA?
All types of jobs, including domestic helpers and industrial workers, are eligible for the FOMEMA Medical Checkup.
What is the PERKESO Health Screening Programme (HSP)?
The Health Screening Programme (HSP) is an incentive provided by PERKESO (SOCSO) as part of its effort to promote healthy lifestyles and to identify groups of workers at high risk of developing non-communicable diseases. Through this programme, health screening is offered free of charge to workers in order to drive awareness among them of the importance of health screening for the early detection of non-communicable diseases.
What is HSP 3.0?
HSP was first offered by PERKESO in 2013. As of 2022, close to 600,000 workers have benefited from the programme by undergoing health screening under HSP.
In 2023, PERKESO introduced HSP 3.0 to enable workers to obtain an up-to-date picture of their health status, particularly in the post-COVID-19 period. In addition, PERKESO now offers improved health screening parameters that detect a wider range of non-communicable disease risk factors.
How long is the HSP 3.0 implementation period?
HSP 3.0 is offered from 16 May 2023 until 31 December 2023. All eligible workers are therefore encouraged to undergo health screening under HSP 3.0 within this period.
What are the objectives of HSP?
a) To build worker awareness of non-communicable diseases and encourage a healthy lifestyle culture.
b) To detect non-communicable diseases at an early stage.
c) To prevent disability and premature death caused by non-communicable diseases.
d) To prevent the reduction or loss of income sources caused by non-communicable diseases.
e) To strengthen the nation’s human capital and productivity through a healthy working population.
What are the eligibility criteria for HSP 3.0?
The eligibility criteria for HSP 3.0 are as follows.
a) Malaysian citizen
b) Aged between 40 and 59 years
c) An active PERKESO contributor under the Invalidity Scheme, that is, having at least one (1) recent contribution in the current year.
d) Having at least 12 months of contributions under the Invalidity Scheme.
A worker has previously undergone health screening under HSP. Is that worker eligible under HSP 3.0?
HSP 3.0 once again offers free health screening to workers who meet the eligibility requirements. All workers, including those who have previously undergone health screening under HSP from 2013 until 15 May 2023, are eligible to undergo health screening under HSP 3.0.
However, the worker must meet all of the stipulated eligibility criteria.
What types of health screening are offered under HSP 3.0?
There are two (2) types of health screening offered under HSP 3.0.

How do I check eligibility for HSP 3.0?
Eligibility for HSP 3.0 can be checked on the SEHATi website (sehati.perkeso.gov.my) under the My HSP Status menu. Workers may also check their eligibility through the SEHATi app, which can be downloaded free of charge from the Google Play Store or the Apple App Store.
How do I get screened under HSP 3.0?
- Download the SEHATi app free of charge from the Google Play Store or the Apple App Store.
- Register as a user in the SEHATi app.
- Complete the Pre-assessment and DASS-21 questions to check your HSP eligibility.
- Select a package for the standard health screening.
- Book a health screening appointment with an HSP Panel Clinic through the SEHATi app.
- Attend the clinic according to the appointment that has been set.
- After completing the standard health screening, female workers who are interested in undergoing a mammogram examination must also book an appointment with an HSP Panel Mammogram Centre through the SEHATi app.

Where can HSP 3.0 be carried out?
HSP 3.0 must be carried out at a panel clinic registered under HSP. The list of HSP Panel Clinics is available through the SEHATi app and the SEHATi portal at sehati.perkeso.gov.my.
Workers must first book an appointment with an HSP Panel Clinic through the SEHATi app before attending for HSP 3.0.
What happens during the health screening session?
Before undergoing the health screening, workers must not consume any food or drink for at least 5 to 6 hours beforehand. The worker’s details and eligibility will be re-verified by the clinic before the screening is carried out.
During the standard health screening session, the clinic will conduct a physical examination covering vital signs, a review of family health history, and blood sample collection. A pap smear sample will also be collected for female workers. However, the pap smear test is offered on an optional basis and is not mandatory.
Blood samples and pap smear samples (if any) will be sent to an HSP Panel Laboratory for further testing.
How is the mammogram examination carried out?
The mammogram examination may only be carried out after the worker has completed the standard health screening. The worker must book an appointment for the mammogram examination with an HSP Panel Mammogram Centre through the SEHATi app.
However, the mammogram examination is offered on an optional basis and is not mandatory.
How are the health screening results obtained?
Workers will receive their standard health screening results within 7 to 14 days. Workers can download their health screening results through the SEHATi app once these have been approved by the HSP Panel Clinic.
If a worker has still not received their health screening results within that period, the worker should contact the HSP Panel Clinic for further checking.
Workers will also receive an appointment date for an advisory session through the SEHATi app or via a phone call from the HSP Panel Clinic.
Mammogram examination results can also be accessed through the SEHATi app, and workers will receive an appointment date for an advisory session for this examination from the same HSP Panel Clinic.
Can a worker’s health screening results be accessed by external parties?
No. Health screening results can only be accessed by the worker through the SEHATi app, and those results will not be disclosed to any external party.
Can the health screening be carried out at the employer’s premises?
Yes. Employers may collaborate with an HSP Panel Clinic to coordinate health screening at the workplace in order to encourage more workers to come forward for screening.
However, health screening results can only be accessed by the worker.

I have never undergone health screening under HSP before. Am I still eligible to undergo health screening under HSP 3.0?
Yes. A worker may undergo health screening under HSP 3.0 provided they meet the HSP 3.0 eligibility criteria.
Can I undergo more than one health screening under HSP 3.0?
No. A worker may only undergo health screening once under HSP 3.0.
I underwent health screening under HSP in March 2023. Am I eligible for HSP 3.0, which begins on 16 May 2023?
Yes. A worker may undergo health screening under HSP 3.0 in the same year provided they meet the HSP 3.0 eligibility criteria.
I underwent health screening under HSP in 2013, but I did not have a mammogram examination. Am I eligible to undergo health screening under HSP 3.0, including a mammogram examination?
Yes. A worker may undergo health screening including a mammogram examination under HSP 3.0 provided they meet the HSP 3.0 eligibility criteria. However, the mammogram examination may only be carried out after the worker has completed the standard health screening.

How can registration as a PERKESO Health Screening Programme Service Panel be done?
For existing HSP panels, migration from the SEHAT Portal to the SEHATi Portal is required as part of the HSP 3.0 implementation. Please visit the SEHAT Portal (sehat.perkeso.gov.my) to activate your account on the SEHATi Portal.
For new HSP panel registrations, applications can be made online through the SEHATi Portal. HSP panels must agree to the Terms and Conditions of the HSP 3.0 Agreement.
How do I withdraw as an HSP Panel?

SST (Frequently Asked Questions)
1. What is SST?
Sales and Service Tax (SST) is a consumption tax implemented by Jabatan Kastam Diraja Malaysia (JKDM). It applies to certain goods and services at prescribed rates. In the healthcare industry, SST at 6% (Group I) will be charged on healthcare services provided to non-citizens (foreign patients).
2. Why is SST being implemented in healthcare services?
SST is implemented in line with the directive from JKDM to ensure proper taxation, compliance, and fair contribution from non-citizen (foreign) patients receiving medical treatment in Malaysia. It does not apply to Malaysian citizens.
3. When will SST implementation begin?
SST will be implemented in phases across Salam Alliance entities, based on revenue thresholds:
– Salam Alliance (SA) will be the first to implement SST starting 01 September 2025 at 6% (Group I), applicable to healthcare services provided to non-citizens (foreign patients).
– Other entities will follow according to their registration thresholds and JKDM timelines.
4. Which healthcare services are subject to SST?
All healthcare-services rendered to non-citizens (foreign patients) in Salam Alliance entities are subject to a 6% SST.
Example: If a foreign patient visits for consultation, treatment, or procedures, the bill will include a 6% SST charge.
However, the sale of goods such as medical aids (e.g., medical equipment or devices) provided to patients is not subject to service tax.
5. How will SST appear on my bill?
The SST amount will be displayed and included in the total bill or receipt issued to the foreign patient.
6. Does SST apply to Malaysian citizens?
No. Healthcare services provided to Malaysian citizens are exempted from SST.
SST only applies to non-citizens (foreign patients).
7. Which Salam Alliance entities are affected first?
SST implementation will begin with Salam Alliance (SA) on 01 September 2025 for the following Klinik As-Salam branches:

8. What is the SST rate for Salam Alliance?
The SST rate applicable to healthcare services under Group I is 6%.
9. Are there any exemptions or special cases?
– Malaysians citizens: Always exempt from SST
– Foreign patients (non-citizens) paying by cash or through Panel (TPA/DP): Subject to 6% SST, except for FOMEMA.
– Dependents of Malaysian citizens (who are foreign nationals): Subject to 6% SST, as the exemption only applies based on the patient’s nationality, not the spouse/family link
– Permanent Residents (PR holders): Considered non-citizens and therefore sub
– Mixed household cases: Only the non-citizen patient’s bill will include 6% SST. Malaysian family members remain exempt.
10. If there are refund cases, will the SST amount be refunded?
Yes. Any refund will include the SST amount and will be returned to the foreign patient.
11. How about promotional or discounted packages purchased by foreign patient?
The payment for promotional items or packages must include SST charges.
